We have formally entered the tax season which means 1095 forms. If you lost your form, do not have access to a printer, your dog ate it and you are in Washington State. Then contact Suzie Health Solutions .  If it is for a paid plan off of the Washington Healthplanfinder then it literally takes less than 5 minutes to recreate another one. If you walk into our office at 1630 N Wenatchee AVE, Suite 18; Wenatchee then we will even print  a copy for you. We do not charge and this is just one of the perks that we do. If you have a 1095 for Apple Health. This takes a little longer. We do need an email address and the Healthcare Authority has to mail it to the address listed. I do dozens of these every years.
